We help older adults and individuals with disabilities remain safely in their homes by connecting eligible community members with services that improve their health, independence, and quality of life.We are seeking a highly motivated Temporary Community Outreach Coordinator to expand awareness of our services throughout San Diego County while supporting several strategic outreach initiatives.Position SummaryThe Community Outreach Coordinator will be responsible for building relationships with community organizations, healthcare providers, cultural organizations, senior-serving agencies, and referral partners throughout San Diego County. This role is ideal for someone who enjoys networking, organizing outreach efforts, and making a meaningful impact in underserved communities.This is a temporary position created to accelerate several outreach initiatives and community partnerships.Responsibilities
Identify and build relationships with community organizations and referral partners.
Schedule and attend community meetings, networking events, health fairs, and educational presentations.
Conduct outreach to:
Community Centers
Cultural and ethnic organizations
Senior Centers
Faith-based organizations
Federally Qualified Health Centers (FQHCs)
Medical practices
Nonprofit organizations
Social service agencies
Coordinate meetings for leadership with prospective partners.
Track outreach activities and maintain detailed records within our CRM.
Follow up with organizations after meetings and events.
Distribute educational materials regarding Medi-Cal Community Supports and private duty caregiving services.
Help coordinate community presentations and educational workshops.
Assist with marketing campaigns and special outreach initiatives.
Represent Coast Care professionally within the community.QualificationsRequired
Excellent verbal and written communication skills
Strong organizational and time management skills
Comfortable speaking with professionals and community leaders
Self-motivated with the ability to work independently
Valid California driver's license and reliable transportation
Proficient with Microsoft Office and Google WorkspacePreferred
Experience in healthcare, home care, social services, nonprofit outreach, or community engagement
Familiarity with Medi-Cal, CalAIM, or healthcare referral networks
Bilingual (Spanish, Arabic, Vietnamese, Tagalog, Somali, or other languages common in San Diego County) is a plusWhat Success Looks LikeSuccessful candidates will:
