*What You’ll Be Doing*
* Provide high-quality, patient-centered primary care to a diverse, multilingual population
* Perform physical exams, comprehensive assessments, and develop individualized treatment plans
* Diagnose and manage acute and chronic illnesses such as diabetes, hypertension, asthma, and depression
* Prescribe medications and order diagnostic tests per scope and protocol
* Provide preventive and wellness care, including immunizations, screenings, and patient education
* Counsel patients on disease prevention, health promotion, and lifestyle modification
* Collaborate closely with physicians, nurses, case managers, and behavioral health staff to improve outcomes and reduce disparities
* Document care accurately and efficiently in the electronic health record (EHR)
* Participate in quality improvement initiatives and population health programs
* Use bilingual or trilingual skills (English, Cantonese, and/or Mandarin) to deliver culturally responsive, patient-centered care and build trust with the community
*What You Bring to the Role*
* Certified Physician Assistant (PA-C) or licensed Nurse Practitioner (NP) with a valid California license
* Graduate of an accredited PA or NP program
* At least 1 year of clinical experience in primary care, family medicine, or community health (2+ years preferred)
* Current DEA registration and BLS certification
* Strong clinical judgment, communication, and teamwork skills
* Commitment to Precision Health Center’s mission of providing equitable, high-quality healthcare to underserved communities
* Bilingual or trilingual in English, Cantonese, and/or Mandarin highly preferred
*Schedule & Work Details*
* Day shift | Monday through Friday
* Flexible hours (30–40 hours per week)
* In-person at our Chinatown, Los Angeles clinic
